Involvement in Healthy Future Sask takes anywhere from one (1) to three (3) hours of your time and involves:
There are two options to complete the health and lifestyle questionnaire for your convenience.
First, you can join the study and complete the questionnaire online by clicking HERE.
If you prefer you can contact us to receive a study package in the mail, along with a prepaid return envelope, and fill out a paper-based study questionnaire. After completion of the questionnaire, an optional visit to attend one of our study centres so that we can take some measurements and collect a biological sample will be arranged. Please note that the HFS team will be reaching out to you when Study Centre appointments become available. In addition, with Healthy Future Sask being a decades-long population health study, we will follow up and check-in with you periodically throughout the next 50-years in a similar fashion.
