Participation includes

• Completion of a health and lifestyle questionnaire (approximately 30 minutes); and

• If you choose, a visit to a study centre to provide physical measures and/or a blood sample or a saliva sample by mail. Physical measures include: Height, weight, blood pressure, grip strength and body fat percentage.  (approximately 30 minutes).

Your information with us is private, safe and secure.

How to Complete the Questionnaire

* The questionnaire will gather information about your overall health and lifestyle including but not limited to family medical histories, personal medications, and self-reported physical attributes. Having family medical history, your medications with DINs, and a measuring tape in hand will facilitate your questionnaire completion in one sitting.

You have two options:

1. You can join the study and complete the questionnaire online; or

2. Contact us to receive a paper questionnaire in the mail, along with a prepaid return envelope.

Your Participation Journey

1. After You Enroll

   • Once you complete the questionnaire, the HFS team will contact you when Study Centre appointments become available.

2. Ongoing Participation

   • As a long-term health study, we will invite participants to complete follow-up questionnaires and provide samples every 3–5 years to track health changes over time.
